Manual Therapy Technique of the Week - Graston Technique 'Pin and Stretch' to the Distal Quad
Joe Heiler PT, CSCS This patient underwent arthroscopic medial meniscus repair R knee approximately 8 weeks ago. He was NWB for the first 4 weeks with a tremendous amount of swelling and a great deal of stiffness when he first came in. I feel like I maxed out his extension from treating the posterior knee the first few visits, so I figured I take a shot at the quad next. This is the 3rd treatment to this area and looking back in the notes he has gained about 10 deg. of extension and his prone knee flexion has improved from 8" heel from buttock to actually touching (as you will see in the video). It's a great spot to treat on your post-surgical patients. . . .

Don't Forget the SC Joint
Patrick Ward LMT, CSCS Soft tissue therapy and training of the shoulder has grown in leaps and bounds over the past decade. While some may still be limiting their treatments to isolated soft tissue techniques at the site of pain and theraband exercises for internal and external rotation most have moved on to include a more holistic and full body approach. The role of the scapula and thoracic spine are always discussed in the movement of the glenohumeral joint as limitations in movement here will translate to aberrant shoulder movement. It is commonly talked about now that the thoracic spine needs to have its mobility restored so that the scapula can display optimal stability thus allowing the glenohumeral joint to produce safe and efficient mobility. One joint that seems to constantly get overlooked in the equation is the sternoclavicular (SC) joint. . . .

Case Study - Hip Flexor Strain
Joe Heiler PT, CSCS Here is case study #2 looking at a 15 yo Male athlete who injured his L 'hip flexor' (that was the official diagonsis, not mine!) during a basketball scrimmage this summer. Of course football was to start in 3 weeks and he was the starting JV quarterback. I chose this type of case in particular because this is a tricky area to treat in my opinion, and we get lots of questions about it here on the site. Hopefully I explain my SFMA logic well enough to follow, you'll get to see a few DNS exercises thrown in, and some strength training/run mechanics progressions to get him back up to speed. . . .

Self Talar Mobilizations

Updating the Multi-Segmental Flexion Corrective Exercises: Hip Flexion Mobility/Stability
Joe Heiler PT, CSCS It's been a long time since I first posted the MS flexion pattern corrections on the site, and since that time I like to think I've refined things a bit and become more efficient. In this post, I'm going to discuss the hip flexion component of the greater flexion pattern. The self-mobility correction is based off Mulligan's mobilization with movement, and is done with a band giving traction to the hip capsule (thanks to the Frederick's for that idea). From there it is working through hip flexion following the neurodevelopmental progression. . . .

Manual Technique of the Week - Lateral Taping of the Achilles
Joe Heiler PT, CSCS I recently purchased the 'Mulligan Taping Techniques' DVD and have to say it was well worth the money. We've discussed many of these techniques previously as they've applied to holding joint mobilizations, but in the DVD there are many techniques to reposition tendons to alleviate tendinopathy symptoms and even the muscle belly in certain instances for muscle strains. I'll be showing a few of those techniques in the coming weeks, and a few more I've come up with using the Mulligan principles but were not on the DVD. That's the great thing about the DVD in my mind, you can take his ideas and run with them when it comes to addressing the specific needs of each individual patient. . . .
